Privacy Statement

‘EYEREUM ophthalmic clinic (hereinafter referred to as “Hospital”)’ cares about customer’s personal information, and abides by the “Act on Promotion of Information and Communication Network Utilization and Information Protection”. The Hospital shall inform you of the purpose and the method of using personal information you provide, and what measures are being taken to protect the personal information from the Privacy policy. The Hospital shall notify you through the website announcement (or individual notice) when revising the Privacy policy.
  • Collected Items of Personal Information

    The Hospital collects the following personal information for membership application, counselling, Services application, and others.

    • Collected items : name, log-in ID, password, e-mail, contact (telephone number, mobile phone number)
      - The following information may be automatically generated and collected during the process of using the Services or the process of providing the Services : Services usage log, access log, cookie, access IP information

  • Purpose of Collection and Use of Personal Information

    The Hospital uses the collected personal information for the following purposes. All information provided by the user shall not be used for any purpose other than that required for the following purposes, and will be subject to prior agreement if the purpose of the use is changed.

    • Homepage Membership Information
      Appointment through the homepage, provision of appointment inquiry and membership-only service, Hospital news via e-mail, information on diseases, questionnaire
    • Use in marketing and advertising
      delivery of advertising information such as events, Identification of access frequency, or statistics for the use of Services by members
  • Period of Retention and Use of Personal Information

    In principle, the Hospital will destroy the information without delay after the purpose of collection and use of personal information is achieved. However, even if the purpose of the provision is fulfilled, in case of necessity to preserve for the hospital news, disease information, and survey through e-mail in accordance with laws and regulations such as commercial law, and your personal information can be stored for the period set below.

    The period of use and destruction of personal information is as follows.

    • Period of Retention and Use of Personal Information
      3 years from the date of collecting personal information (it shall be immediately destroyed when requested for withdrawal of personal information, or withdrawal of membership)
    • Procedure of Destruction
      The information entered by the member for membership application and others is transferred to a separate DB (in the case of paper, separate document box) after the purpose has been achieved, and will be discarded after being stored for a certain period of time according to the internal policy and other relevant laws and regulations (refer to the period of retention and use). Personal information transferred to a separate DB will not be used for any other purposes unless otherwise required by law.
    • Method of Destruction
      - Personal information stored in the form of electronic files is deleted using a technical method that forbids reproduction of the record.
      - Personal information printed on paper is shredded by crusher or destroyed by incineration.
  • Consignment of Collected Personal Information

    The Hospital will not entrust your information to outside companies without your consent. If such a need arises in the future, it shall be notified to the customer of entrusted person and entrusted business contents, and the prior consent shall be obtained if necessary.

  • Rights of Users and Legal Representatives and Exercising Method

    User may view or modify his or her registered personal information at any time, and may request to terminate the membership.

     

    By clicking ‘Change the Personal Information’ (or ‘Edit the Membership Information’) in order to view or modify the personal information of users, and clicking “Cancellation of Membership” in order to terminate the membership (consent withdrawal), you can view, modify, or withdraw from after the process of identity verification process.

     

    Or, if you contact the person in charge of personal information management by letter, phone, or e-mail, immediate action will be taken without delay.

     

    If you request correction of errors in your personal information, your personal information shall not be used or provided until completing the correction. Furthermore, if inaccurate personal information is already provided to the third party, result of the correction shall be notified without delay so it could be revised.

    The Hospital treats personal information that has been terminated or deleted at the request of the user as described in the “Period of Retention and Use of Personal Information Collected by the Hospital”, and is prohibited from viewing or using for any other purposes.
